Agreement for Coaching and Program Services

Welcome to Peacemaker Coach, a professional Coaching and Teaching practice. This document and attachments constitute a contract between us ("Coaching Agreement"). You should read it carefully and raise any questions and concerns that you have before you submit it.

To enroll, you must read and approve the terms of this Agreement in order to participate in the Parent Success Program.

Services:
Coaching services vary depending on your needs and desired level of support. They consist of either group or individual coaching by teleconference or in-person. The costs vary with the amount of time you request. To be successful requires your commitment as the Client and group member to take responsibility for your participation and the outcomes that result from it. You'll need to be open and honest about your issues and needs, keep agreements and stay committed to your goals. My role as Coach is to employ my expertise as a mentor, supporter, taskmaster, motivator, and sounding board. The balance between support and challenge is the key to keeping you focused, holding your accountable for practice, and championing your results--all of them!

Nature of Relationship:
The Client understands that the coaching relationship is not psychotherapy, psychological counseling, or any type of therapy; nor is it a substitute for these services. In the event the Client feels the need for professional counseling or therapy, it is the responsibility of the Client to seek a licensed professional who can provide these services. If the Coach believes that such services will be valuable to the Client, the Coach will recommend them.

Confidentiality:
I protect the confidentiality of the communications with my Clients. I will not voluntarily divulge that I am in a coaching relationship with you without your written permission, unless I am required to do so by law.

Many sessions are conducted in teleconference groups. You agree to maintain the confidentiality of all information communicated to you by other coaching clients and by your Coach. However, results are often enhanced when clients discuss their coaching relationship with trusted colleagues and friends. It's fine to have these types of discussions, but you are expected to be very careful not to share any information that would allow others in the group to be identified. One way to decide how and what to discuss is to think about how you would feel if someone else in the group was discussing you.

As you are probably aware; it is impossible to protect the confidentiality of information which is transmitted electronically. This is particularly true of E-mail and information stored on computers connected to the internet (unless you use encryption and other forms of security protection); and if you use a cordless or cell phone, someone with a scanner could hear you talk.

Feedback:
If, at any time, you feel that your needs are not being met or you are not getting what you want out of the Program, please tell me. We can discuss your needs and adjust your participation as needed. We will continue to work on the goals that you define unless you want to stop, which we will do whenever you ask.

Mutual Nondisclosure:
The Coach and Client mutually recognize that they may discuss future plans, business affairs, customer lists, financial information, job information, goals, personal information, and other private information. The Coach will not voluntarily communicate the Client's information to a third party. In order to honor and protect the Coach's intellectual property, the Client likewise agrees not to disclose or communicate information about the other members of the coaching group. The Client also agrees NOT to give the Peacemaker Parent Success Program bridge numbers to anyone not enrolled in the Parent Success Program. In the event the Coach wishes to use any class materials, including audio or written material, the Coach will secure Client permission by signed release waiver.

Meeting Schedule:
45 to 60 minute Topic Calls are scheduled each Monday and 30 minute Support Calls are scheduled Tuesdays, Wednesdays, Thursdays, and Fridays during the program period. See Class Schedule for actual times. While participants are encouraged to attend any or all support calls (day and/or evening times), participation in Topic calls will be held to the participant's respective enrollment choice (either day or evening program.) In addition, the Coach will conduct one-on-one laser coaching sessions during specific times each week. Clients wishing to discuss issues privately are invited to schedule a 10 to 15 minute laser session during this “set aside” time at no additional cost. In the event the Client is unable to attend a meeting, all meetings will be recorded and made available to the Client for review at his or her convenience.

Call Procedure:
The Client will call the Peacemaker Parent Success Program bridge line at the prearranged time and telephone number as scheduled. The Coach will pay for the program bridge; and the Clients will pay for the call into the program bridge.

Cancellations:
As all meeting calls are recorded and made available to all Clients in the group, no refund will be made or credit given for any missed meetings. In the event a meeting must be cancelled, the Coach will make reasonable efforts to reschedule the meeting.

Payment Procedure:
The Coach is paid in advance of each program. Payment may be made by the Client by check, Visa, MasterCard, or American Express. Program services must be paid in advance, or participation by the Client will not be authorized. Services requested by the Client, in addition to scheduled Program calls and one-on-one laser coaching calls, will be billed at $75.00 per ½ hour. Payment for additional services are billed separately to the Client immediately following the rendered service. Payment is due within 7 days of invoice date. Any changes to this procedure must be mutually agreed upon in writing.

Termination: Within the first 2 weeks of the 13 week Program, the Client may drop the program for any reason and will receive a full refund of Program fees paid. After the second week, Clients wishing to drop the Program are not guaranteed a refund. To drop the program after 2 weeks, the Client must provide the Coach a written notice by E-mail or USPS.

Dispute Resolution
Any controversy or claim arising out of or relating to this Agreement, or the breach of this Agreement, shall be settled by arbitration, which will occur via telephone by an arbitrator that we mutually agree upon. The costs of the arbitration shall be shared equally between the parties.
